In order to help speed up the enrollment process, please have the following:
*PREVIOUS SCHOOL RECORDS
Withdrawal record from previous school attended required; Last report card from previous school attended.
*SOCIAL SECURITY CARD
We must have a copy of the enrolling student's Social Security Card.
*PROOF OF RESIDENCY
To verify, upon enrollment, that the student and/or either parent is a resident of the district. Clyde CISD requires this information. MUST be a Gas, Water or Electric Bill, House Contract, Lease Agreement (A utility bill is preferred).
*BIRTH CERTIFICATE
We must have a copy of the enrolling student's leagal Birth Certificate.
*SHOT RECORDS
Out of State students MUST HAVE SHOT RECORD at the time of enrollment. Shot records must be provided within the first 30 days of enrollment.
*PARENT/GUARDIAN NAME & PHONE NUMBER
Clyde CISD will record the name, address and date of birth of the person enrolling a child. The child must be enrolled by the child's parent or by the child's legal control of the child under a court order. A copy of the person's drivers license that is enrolling a child may be asked.
